Why is coordinated care important?
The best HMOs and PPOs have primary care doctors, specialists, and other health care providers that communicate and work together to provide the best care for you.
How can health plans make sure that you receive coordinated care?
Highly rated plans work with their doctors and medical groups to set up systems to effectively handle and communicate information properly to ensure that your regular doctor is up to date on the status of your health needs and your health care.
What do the stars mean?
The scores show the percentage of health plan members who reported that their doctor was up to date about the care they got from other doctors. A higher score means more members said their doctors worked together to coordinate their care.
The scores are based on information from a survey of a sample of at least 100 health plan members in 2023.
